I hereby grant permission to Alaska to use and incorporate my name, likeness, image and voice (alone or together with other audio, visual or video materials) derived from any photographs, video footage or other recordings that are taken during the Alaska Airlines event. If I am bringing one or more guests to the event, I represent that my guests have agreed to grant the same permission to Alaska Airlines on their behalf, and hereby grant such permission.

• I waive all rights of publicity and rights of approval for myself and my guests.
• I acknowledge that I am over the age of 21, and my guest(s) are over the age of 21.
